Membership

A person holding a Hong Kong Identity Card or a Certificate of Exemption issued under the Registration of Persons Regulations (Chapter 177 of the Laws of Hong Kong), except a person whose Hong Kong Identity Card was issued by virtue of a permission to enter or stay that he or she has been granted and such permission has expired or is no longer valid; or a child under the age of 11 who is a Hong Kong resident;
Persons who agree to join the Electronic Health Record Sharing System; and
Hong Kong residents aged 6 or above who live or work in Yuen Long District.

Membership Registration Procedure

If you wish to become a member, you must bring the following documents to our center for registration:
- Hong Kong Identity Card
- Proof of residential address with the applicant’s name; or
- Employment certificate with their name, company name and company address
Applicants who have the following medical fee exemption certificates, please bring along the following documents for verification by the centre staff:
- Comprehensive Social Security Assistance Scheme
- Old Age Living Allowance (75 years and above)
- Elderly residential care service voucher (Level 0 persons)
- Certificate of Medical Fee Waiver from Hospital Authority

Remarks: After membership registry, “Health risk factors assessment” service will be arranged according to the actual situation of the day. Wong Tai Sin District Health Centre reserves the right to make arrangements and final decisions.
